You can use this template if you:
 Slaughter and dress farmed mammals, ostriches or emus; and
 Have a low throughput; and
 Process meat for human consumption; and
 Operate a fixed or mobile micro abattoir premises; and
 Process meat for New Zealand and/or export to countries that do not require Official Assurances.

This application form is for manufacturers exporting new vehicles and/or machinery from all countries to New Zealand and wish to have an MPI-Approved New Vehicle or Machinery System. This form also provides guidance around reducing the risk of biosecurity contamination within the vehicle or machinery supply chain.

In the event that a food safety issue arises with our products (insert company name) will protect public health by facilitating the efficient, rapid identification and removal of unsafe food from the distribution chain and, by informing consumers (where necessary) of the presence in the market of a potentially hazardous food.

This template is intended for small to medium sized operations which manufacture cheese for the domestic market (Australia and New Zealand only) in accordance with the permitted methods of processing as described in the Food (Milk and Milk Products Processing) Standard 2007

It also provides instructions on how to complete this RMP template. The person completing this template must read these instructions while completing the form to ensure that correct information is provided.
